So What’s Your Skin’s Issue?
With FaceTime, Skype, selfies, and magnifying mirrors, we’re looking at ourselves quite a bit more these days! While we should all celebrate what makes us beautiful, we can’t help but see what can be improved and what may be changing in a way that we don’t like. It’s natural to do so!
In looking at yourself, you may start to see changes in your skin that reflect the toll taken by various skin conditions (rosacea, acne, eczema), lifestyle and environmental factors (sun exposure, smoking, stress), genetic factors (pore size, moles, dark circles, angiomas, skin growths), hormonal changes (pregnancy, menopause, oral contraceptives), and the aging process (wrinkles, folds, sagging skin). Taken together, the cumulative effects of all the these processes, can make one look less vibrant, weathered, tired, or older than they’d like. At Steele Dermatology, we understand.
- + Wrinkles
When we think about wrinkles, we’re typically talking about those fine lines that you see around the eyes (crow’s feet), around the mouth (lip lines), or on the forehead. The causes of these fine lines is actually multifactorial. They develop because of the way we move our face over the years, sun and oxidative damage to the skin that makes it thinner, and the gradual loss of underlying tissues the make our skin look firm and supple. Treatments should address the various causes of the wrinkles. Botox is an awesome treatment for wrinkles to soften your movements and not “pull” so hard on your facial muscles. Not only will that give you an immediate result but will also prevent future wrinkles. The optimal time for Botox is when wrinkles are still subtle and when they are mostly present during movement with only a shallow wrinkle visible when you’re not moving your face.When wrinkles are a bit more advanced, superficial fillers are an excellent option with immediate gratification and long lasting results. A filler is a substance (usually a hyaluronic acid) that can be placed right under the skin to physically fill in the wrinkle so that the skin texture is smooth. Examples include Restylane Silk and Belotero Balance due to their consistency. Fillers generally replace lost collagen and other underlying supporting tissues that keep our skin looking supple and firm.
Additionally, everyone who is starting to see wrinkles (basically everyone) should be using skin care products that will maintain and rebuild collagen in the underlying skin layers and combat oxidative and UV damage by the environment and sun. Antioxidant formulations, retinoids/retinols, growth factors and sunscreen are the must haves!
- + Creases
Creases are when wrinkles become deep and rather “set-in”. These are generally areas where you move your face in a way such that you pull very hard in one area that creates a crease over time. A good example is the “11 lines” in between the eyebrows. Some people will get very deep creases there or possibly one very deep crease because they pull harder on one side vs another. Botox is very helpful in minimizing the appearance of a crease, but it’s usually not adequate. A filler is a substance placed in the superficial layers of skin to fill in that crease and elevate the depressed area to match that with the surrounding skin. Fillers give immediate and long lasting results. Optimal products include Restylane and Juvederm.
- + Folds
The most common fold that we talk about is the nasolabial fold, otherwise known as the “parentheses lines”. They extend from the point where our cheek meets the side of our nostril down to the angle of the mouth. People have varying degrees of nasolabial folds, but they do tend to get more prominent as we get older. Folds represent volume loss, i.e. where the supporting structures of our skin (bone, collagen, fat) gradually disappear over time and consequently, the skin starts to sag without an adequate infrastructure upon which to lay. This not only happens in the nasolabial folds, but also around the eyes, in the temples, in different portions of the cheek, and off the face as well. As volume loss (can link to my paragraph about this in the skin section) occurs and folds become more prominent, people usually complain that they looked “sad”, “tired” or “worn out” and generally like their skin and face looks less vibrant and youthful. Without volume and “lift” under the skin, the face appears to be flat and sagging or heavy. A healthy and natural degree of added volume, skin tightening, and skin rejuvenation can turn everything around. Treatment is two fold:
1) volume must be restored to the supporting tissues and
2) your skin must be stimulated to maintain existing collagen and produce new collagen.
Volume replacement typically involves placement of a deep filler to provide support and lift to the overlying skin. Options include Radiesse, Volume and Perlane. Another route is use a biostimulatory product like Sculptra which is also injected under the skin but causes your own natural collagen production. As the most abundant protein in the body, collagen is essential for healthy skin. These treatments are long-lasting (1-2 years) and help re-store a more youthful, natural and radiant appearance.
There are also some lasers and procedures that can stimulate collagen and give results that are similar to or complement that of fillers, like a deep radio frequency treatment that stimulates deep collagen production. Again, this may be used in conjunction with or even instead of fillers in some cases. This procedure creates a natural, lifted, and supple appearance to the skin and soft tissues.
The proper products are vital to your skincare routine to maintain skin integrity and healthy volume. They should be used to stay ahead of the aging process, and get the most longevity out of your treatments and procedures. Retinoids and retinols and growth factors are the 2 main components to slowing the skin’s tendency to lose collagen and elastic tissue over time, and generate new collagen and produce healthy skin turnover.
- + Bumps and Growths
As you age you can start to see all sorts of skin growths (i.e. lesions that are raised and you can feel), including sebaceous hyperplasia, milia, syringomas, seborrheic keratoses, dermatosa nigra papulosa, etc. The list is very long, but Dr. Steele sees and treats them all.These bumps and growths disrupt the skin texture and interfere with a smooth appearance and feel. The first step to restoring the skin’s texture is removing these lesions which can be done in various ways depending on your skin and lesion types. Treatments include laser removal, cryotherapy (freezing), and chemical peeling.Unfortunately, preventing these lesions is not an option. However, once Dr. Steele removes them, you will typically come in for future treatment when they are small in size and number for maintenance treatments.
- + Spots and Discoloration
From blemishes and post-inflammatory hyperpigmentation to melasma to freckles and sun spots (can link to the paragraphs I have on each in the skin section), skin discoloration and uneven skin tone is a problem that affects almost all of us at one time or another and can severely affect the way that one sees their skin and overall appearance.No matter what the cause of your dark spot, it is an excess of melanin deposition. Under certain circumstances, it will clear up on it’s own (post-inflammatory hyperpigmentation where a dark spot after a pimple, chemical burn, hair bump, or cut) but would take a very long time. In most other cases, like melasma and sun freckles/sun spots it’s there to stay unless you receive treatment.In the case of sun freckles and melasma, you’ll see tan-brown single spots or large patches scattered around the face and other sun exposed areas like the face, shoulders, chest and hands. Sun causes them! Of course, some people get more than others with the same amount of sun exposure depending on how easily your melanocytes are activated. In the case of melasma, sun brings it about but hormones (estrogen primarily) and genetic factors play a large role too. In both cases, treatment must be done in conjunction with absolute sun protection during and after treatment. Although dark spots from post-inflammatory hyperpigmentation aren’t absolutely caused by sun, treatment still requires strict stun protection to get the best results during treatment.

Chemical peels, lasers, and creams, lotions and pads are all great options. Dr. Steele will recommend the combination that will work best for you.
- + Texture
Pore size, rough, dry skin, or oily skin can all contribute to less than optimal skin texture (and appearance). These are issues that do not necessarily have to do with aging, but can just be your innate skin quality or from environmental factors. However, they do tend to worsen over time if untreated. With a combination of appropriate products and treatments Dr. Steele and her staff can make your skin texture smoother and softer.Specialized facials, chemical peels, and microdermabrasion can be incredible tools in achieving optimal skin texture. Such treatments remove the superficial layers of skin that tend to be more weathered and create uneven or rough texture. The underlying layers of skin have smaller pore size and a smoother appearance and texture. Lasers that target the superficial layers of skin also do a remarkable job at smoothing the skin complexion.On a daily basis, we offer a full set of skin care options that range from cleansers, toners, serums, and moisturizers that minimize the appearance of pores and keep the skin in it’s most optimal condition.
- + Hanging, Sagging or Crepey Skin
At some point skin in certain areas can become very thin and inelastic from aging, sun damage, and environmental exposures. Patients often use words like “crepey” and “wrinkly” then pinch the skin to show that it just stays sorta crumpled rather than bouncing back. The most common place for this is around the eyes, under the chin, on the underside of the upper arms, and chest. At Steele Dermatology we have the tools to be able to rejuvenate this skin and produce healthier, stronger and more supple skin. Crepey thin skin requires treatment that thickens the skin layers by with the production of collagen. Lasers that heat the skin layers or provide resurfacing will stimulate new collagen and elastic tissues and also tighten the skin to improve the appearance of thin, crepey skin. The skin tightening effect has a remarkable effect on skin appearance. To take care of your results, Dr. Steele and her staff will recommend products that have UV protection, antioxidant properties, and collagen stimulating properties.

VISIA is an imaging system that allows Dr. Steele to precisely detect the condition of your skin in several categories. You allow the device to take a 3D picture of your face then the system will give Dr. Steele an analysis to review with you which shows your degree of UV sun damage, brown spots, redness, pore size, wrinkles, and texture imperfections. While it can be sometimes difficult to pick out these aspects of skin deterioration individually with the naked eye, VISIA allows us to pinpoint and quantify them, which better targets treatment. Not only can we optimally select treatment, but we can also exactly measure treatment results. VISIA is the at the cutting edge of skin analysis and is the best place to start and finish any skin care treatment plan.

- + INJECTABLES & FILLERS
As the science of injectable fillers has become more and more advanced over the last 10 years, so has Dr. Steele’s skill with them. Fillers are substances that can be injected into the skin to smooth out wrinkles, fill small or large areas of volume loss, balance and re-balance the proportions of the face, and correct minor imperfections. Hyaluronic acids are the most common fillers on the market. Hyaluronic acid is a naturally occurring substance in our bodies, but the commercially derived fillers are synthetic. Although it may seem odd to use a synthetic product, they ensure that we don’t have reactions (like often occurred to collagen), and that we have a reliable product with predictable results and uniform particle size. Dr. Steele likes them for patients because they are versatile and feel very natural in the skin. She can use them superficially or in a deeper plane depending on your needs and the specific product.
Other options include a biostimulatory injectable filler that stimulates your own body to create collagen in the areas injected, and calcium hydroxyapetitie which can be used for deep tissue filling. For some areas of the skin specific injectable fillers are preferred, so see Dr. Steele to see what the best options are for you.When you are getting injectable fillers, your face will first be numbed topically for your comfort. After holding some pressure for a few moments you will immediately see the results and be able to resume normal activities with no “downtime”!

Lasers can be used for almost anything and everything with today’s technology, but in Dr. Steele’s opinion they should be used to do things that cannot otherwise be done, to achieve better results than other technologies or to get results with fewer side effects. Intense Pulsed Light is often called a photofacial and is one of the most revolutionary technologies of recent years. IPL can actually be used to target different conditions, but usually red and brown pigments, which includes broken blood vessels, rosacea, sun damaged skin and brown sun spots. IPL has replaced more aggressive lasers of the past that had more side effects and bruising. Dr. Steele is happy to provide IPL to her patients for immediate skin rejuvenation and continued maintenance of the healthiest and clearest skin.
- + BOTOX
Botox is the branded Botulinum A Toxin product most commonly known for treating wrinkles on the face. While Botox was the first product in this space, there are now two other reputable and equally effective toxins, Xeomin and Dysport. These toxins are all very precisely formulated for safe cosmetic use and have been safely used for nearly 20 years since their discovery by a dermatologist.The toxins have well known interactions with signaling pathways that lead to muscle relaxation, decreased sweating, and as we’re discovering, many other effects that can be beneficial when used in other areas or for other uses. Most commonly, toxins are used to smooth out skin that has hyperactive muscle movement leading to a frowned or tense look. For this purpose, there aren’t other treatments or creams that create the same effect, although toxins can also be used to complement the effects of other technologies in skin rejuvenation. Dr. Steele is a well known skilled injector of toxins and has been a leader in cutting edge treatments using it as well.
